
Longitudinal Functional Movement Study Participant Consent.
INFORMATION ABOUT A UNIVERSITY OF WASHINGTON & HOMININ.AI RESEARCH STUDY
What is this study about?
The purpose of this study is to monitor changes to athletes’ dynamic movement and static balance baselines over a period of several months using commercially-available wearable devices and a companion mobile app, InStep, produced by hominin.ai. We are particularly interested in changes that occur to participants’ movement and balance baselines after an injury event and their return to baseline during recovery.
What will you be asked to do?
Participants will be provided with a link to download the InStep app as part of their on-boarding
Participants should download the app, create a login, and fill out the profile section with as much information as they are comfortable providing
Participants will be asked to complete an intake questionnaire regarding their injury history and sporting history as part of their on-boarding
Participants will be asked to perform a static balance test, a tandem (“heel-toe”) walk test, and a normal walking test
Each test should be repeated 2+ days per week, or as frequently as possible – we suggest before or after each team practice, depending on team preference – the timing will be noted in the in-app survey associated with each session
Static balance tests can be completed alone, or with a partner to start the in-app timer
Tandem walking tests can be completed alone, or with a partner to start the in-app timer
Normal walking tests can also be captured along any walking route in any conditions, participants are encouraged to collect walking data whenever possible
If the participant suffers an injury, we ask that they continue to perform the tests as soon as they are able
In the event of an injury, the team doctor / coach will be asked for an assessment of the injury at time of event and treatment plan, they will also be asked to provide follow-up assessments throughout recovery
This study is expected to run over the course of 4-6+ months, or for the duration of a competition season. Each test will take approximately 2-3 minutes of your time.
How do we collect your data & personal information?
You provide hominin.ai with some of the data we collect. We also collect data through third-party hardware devices such as your phone and smart wearables. We collect data / personal information through:
User registration on our InStep app
Voluntary user-intake survey upon registration on our app
Voluntary opt-in on our app to sharing movement and geospatial data via your phone and smart wearables
Voluntary user input to in-app questionnaires regarding current and historic health status
Your usage or viewing our website via your browser’s cookies
hominin.ai will also receive your data indirectly from the following sources:
Smartphone or smart wearable devices
Why might you want, or not want, to participate?
Some people may feel that participating in a research study is an invasion of their privacy. If you do participate, it is possible that you could trip while walking. It is possible that you may fall over during the static balance or tandem walk tests. You will be able to rest between tests if you experience any tiredness or discomfort. You may refuse to complete any test, at any time, for any reason.
How do we store your data & personal information?
hominin.ai securely stores your data with AWS cloud services.
hominin.ai may keep your data indefinitely to ensure longitudinal insights. If you want us to delete your data, contact us at the email listed below.
Benefits of the Study
Our study may not directly benefit you, however, we hope to provide data on the potential benefits from wearables like AirPods that may be tools used to help people in their recovery from injury.
How will we protect the information you provide?
We will protect your confidentiality. We will store your name and other identifiable information separate from the study data. Access to your identifying information will be limited to your Team Leader, certain members of the study team, or other agencies that may need to audit study records. When we publish the results of this study, we will not use your name. We will not retain your personal identifiers and your data will not be linked to your personal identifiers.
The information and/or samples that we obtain from you for this study might be used for future studies. We may remove anything that might identify you from the information. If we do so, the information may then be used for future research studies or given to another investigator without getting additional permission from you. It is also possible that in the future we may want to use or share study information that might identify you. If we do, a review board will decide whether or not we need to get additional permission from you.
What if you want to stop being in this study, or if the researcher decides you should no longer participate?
You may refuse to participate, and you are free to withdraw from this study at any time without penalty or loss of benefits to which you are otherwise entitled. If you wish to withdraw, please contact the researcher listed on page 1 of this consent form.

Being in this study is voluntary. This means that you can refuse to sign up. It also means that if you do sign up, you can decide to stop being in the study at any time without penalty. If you wish to withdraw, please contact one of the study team.
